FACT Team Position # 961 SIGN-ON BONUS! We are looking to select a compassionate and driven professional to work collaboratively with Central Florida Behavioral Health Network that thrives on stepping up, taking charge of their caseload and, most importantly, taking care of their Clients! POSITION DETAILS:
- Working as part of a multi-disciplinary team to ensure continuity of care to Clients through case management services in a community-based setting.
- Helping FACT Clients link with housing options in the community.
- Participating in community-based events focused on the housing needs of FACT clients.
- Serving the adult population to improve quality of life via linking them and their families to resources, such as housing, and other ancillary social support services.
- Completing assessments, developing recovery plans/natural support systems and providing education.
- Developing, writing, implementing, evaluating and revising overall treatment goals and planning on-going assessment of needs based on input from the Client and family.
- Conducting on-going assessment of behavior, mental status, signs and symptoms of mental health disorders, physical and dental health, substance use, and educational, social, legal, financial, housing, developmental and family functioning.
- Acting as liaison and collaborating with community agencies/institutions and support systems to maintain coordination of the treatment process.
- Collaborating with primary care providers, treatment teams, community providers and potentially local treatment courts.
- Providing on-call crisis intervention services during nighttime and weekend hours as assigned (rotated among the FACT Team).
- Maintaining electronic medical records and updating all documentation entered within established time frame.
- Referring, advocating for and linking Clients and family with additional services as identified.
- Completing all required DLCenters' and FACT trainings/meetings.
- Daily to job duties to facilitate the development of therapeutic relationships with Clients and to facilitate consistent services.
- Other related duties as assigned by FACT Team Leader/Supervisor.

- Bachelor's Degree in human services field from an accredited institution = Required - Knowledge of local community resources = Strongly Preferred - Understanding of community services and experience in a mental health setting = Strongly Preferred - Enthusiasm for group facilitation that develops rehabilitation, recovery and life skills = Strongly Preferred.
- Spanish Bilingual = Preferred - Personal commitment to working with adults that have severe and persistent mental illness disorders = Required - Current State of Florida Driver's License with clean record = Required - Demonstrated abilities in the proper management of time, communication and information = Required - Natural ability to work well with a diverse group of community partners = Required - Comfort with local travel, working in different environments and interacting with Clients, residents, family members, visitors, government agencies/personal and others in a behavioral health setting = Required - Experience with Electronic Medical Records = Strongly Preferred - Experience with Microsoft Office 365 (Including Teams) = Required - Staying up to date on all trainings and vaccinations = Required LOGISTICS & COMPENSATION:
